This test includes fluorescence in situ hybridization (FISH) for high-grade B-cell lymphoma, targeting rearrangement of BCL2, BCL6 and MYC.

Use

This test is a diagnostic test for non-Hodgkin's lymphoma (NHL) that detects primary genetic changes associated with various types of NHL, including high-grade B-cell lymphoma, follicular lymphoma (FL), Burkitt lymphoma (BL), diffuse large B-cell lymphoma (DLBCL) and those with BCL6 rearrangement.

Special Instructions

Indicate the pertinent clinical diagnosis and previous cytogenetic studies on the test request form. Note: This test number is used for tracking FISH specimens to the laboratory.

Limitations

Molecular mutations or chromosomal alterations not targeted by the probe included in this profile will not be detected.

Specimen Requirements

Specimen

Whole blood, bone marrow, CSF, lymph node, fixed-cell pellet from a cytogenetic analysis, slide with metaphase and/or interphase nuclei, bone marrow touch preparation slides, paraffin slides or block are acceptable for testing.

Volume

5 mL whole blood (pediatric), 3 mL bone marrow, fixed-cell pellet from a cytogenetic analysis, 6 slides with metaphase and/or interphase nuclei or 6 touch preparation slides or 6 slides 4- or 5-micron thick with tumor

7 mL blood (adult), 1-3 mL bone marrow, five slides from cytogenetic analysis or seven slides from paraffin block, one tube fixed pellet, 4 touch prep slides

5 mL whole blood (pediatric), 3 mL bone marrow, fixed-cell pellet from a cytogenetic analysis, 6 slides with metaphase and/or interphase nuclei or 6 touch preparation slides or 6 slides 4- or 5-micron thick with tumor

Minimum Volume

1 mL whole blood (pediatric), 1 mL bone marrow, fixed-cell pellet  from cytogenetic analysis, 3 slides with metaphase and/or interphase nuclei or 3 touch preparation slides or 3 slides 4- or 5-micron thick with tumor (Note: Minimum volumes may not allow for repeat testing.)

1 mL blood, 1 mL bone marrow, and four- to five-micron FFPE slides, 5 touch prep slides

Container

Green-top (sodium heparin) tube; pediatric Vacutainer® is optimal or lavender-top (EDTA) tube  (suboptimal). For samples with shared karyotype (510999) and any FISH study in the same tube, EDTA is not recommended due to the growth inhibition in cell culture. Adjust tube size to sample volume to avoid heparin toxicity.

Storage Instructions

Transport to the testing laboratory at room temperature, use of a cool pack or a Labcorp transport kit is acceptable. Do not allow samples to overheat or freeze.
