Our staff is committed to providing a professional environment for each patient during the specimen collection process.

LabCorp Patient Service Centers provide routine adult and pediatric specimen collections for diagnostic testing and screening. In addition to routine blood and urine specimen collections, many of our centers offer other procedures such as urine drug screens, paternity collections and breath alcohol testing. LabCorp's skilled staff works diligently to provide each client prompt and professional service.

When visiting a Patient Service Center, individuals should bring the following information with them:

The LabCorp test request form from a health care professional requesting laboratory testing
A current insurance identification card (Medicare, Private Insurance or HMO/PPO)
A photo ID for urine drug screens or paternity collections (e.g., driver's license, employee identification badge)

Minors must be accompanied by their parent or legal guardian.

If you are bringing a minor to a Patient Service Center and you are not the legal guardian, you will need to bring a notarized statement signed by a parent or legal guardian authorizing you to give permission for the collection and billing of the tests ordered by the minor's physician.
